Please be sure to bring one of the following to the UNT Passport Office when applying for a U.S. Passport.

  • Original or Certified Birth Certificate issued by the city, county, or state of your birth. Hospital certificates and photocopies will not be accepted. Certified copies of Texas birth certificates may be obtained from any county clerk's office in the state. Birth certificates must include the full name, date of birth, and place of birth of the applicant as well as both parents' full names. The issuing authority's seal and signature must be visible.
  • Certificate of Naturalization
  • Certificate of Citizenship
  • Undamaged U.S. Passport Book or Card 

Citizenship documents will be mailed, along with Form DS-11 and payment, to the Passport Processing Facility. Citizenship documents will be returned  to you via USPS within the same time frame as your new passport.

 

 

Have an abstract version of your Texas birth certificate?

Abstract versions of Texas birth certificates must have a capital letter “I” after the date filed to be accepted as proof of citizenship by the U.S. Department of State.

Abstracts from California are not accepted.
